Departmental Contact Change Form
This form should be used to update directors (DUS, DGS, or other director) and assistants (DUSA, DGSA, or other assistant/coordinator) for Duke departments in the following areas:
- Registrar website and contact lists, including the DARN mailing list and reviewers for Duke University bulletins
- Course Request System

Users will receive an email summary of the request upon submission. Please direct all inquiries regarding the form to bulletins@duke.edu.

Units in the Trinity College of Arts & Sciences should also update their faculty, staff, graduate students, and post-docs through the Trinity Directory Tool.
