This form should be used to update directors (DUSA, DGSA, or other director) and assistants (DUSA, DGSA, or other assistant/coordinator) for Duke departments in the following areas:

Users will receive an email summary of the request upon submission. Please direct all inquiries regarding the form to bulletins@duke.edu.

To add, delete, or modify user security in systems maintained by Student Information Services and Systems (SISS), submit the SISS Access Request Form.

Units in the Trinity College of Arts & Sciences should also update their faculty, staff, graduate students, and post-docs through the Trinity Directory Tool. This database feeds directory information to Trinity unit websites.
